Community Health Choices (CHC) is on the Horizon

Major changes are on the horizon that will change be changing the landscape of long term care.

Community Health Choices (CHC) is a new state initiative to help coordinate care for older persons who are dually eligible for Medicare and Medicaid or for those individuals who qualify for Medicaid long-term services and supports because of the need for the level of care provided by a nursing facility.

Eligible individuals will be required to enroll with a Managed Care Organization (MCO), who are currently being determined by the state. The MCO will in turn begin to coordinate all healthcare for the enrolled individual.  The implementation of CHC has been postponed to January, 1 2018 and there is progress being made daily to work towards a successful launch of the program.

So what else does CHC have to offer? That answer can be found by reading over the Frequently Asked Questions (FAQ) section on the Department of Health website. With a little under eleven months until the program officially rolls out I encourage you all to educate yourself on the upcoming changes.
